I think the idea of a city state as a setting is a great one. Germany was a very fragmented place for a really long time which is why there are so many dialects of it. It would be commonplace for people to travel downriver and not understand anyone when they got off the boat. Generally fantasy deals with characters who have a clear quest and how to get there, what works well in the parts of this story I've read is that the further the search progresses the more enormous the task is and the more hopeless it looks.
